Its harmless appearance can make many forget that the grave companion is undead, brought back by the mist, and can be just as deadly as seemingly more threatening beasts, especially during a Torment.

Characteristics
Simpleton. When the Grave Companion needs to make a strategic decision or interpret an ambiguous order, it has a greater chance of choosing the less effective option or misinterpreting an instruction. Whenever there's a binary choice (attack/defend, chase/flee, etc.), the GM can roll 1d20, and on a 10 or lower the Grave Companion must choose the less advantageous option.
Descycle. Upon being reduced to 0 hit points, the Grave Companion must make a Constitution check with a DC equal to 10 or half the damage taken, whichever is higher. This effect doesn't trigger if it's destroyed by a source that interacts directly with the destruction of undead, such as "Liturgy: Destroy Undead" from an Acolyte of the Spiritual Rite.
Empowerment. Whenever any creature is reduced to 0 hit points or dies within 30 feet of the Grave Companion, it gains 1 Soul token.
Empower Attack. The Grave Companion can spend its Soul points to increase the damage of its attacks by 1 die for every 7 Soul points spent.
Mist Wraith. This undead is native to the Black Mist and is positively affected by it.
Keen Smell. The Grave Companion has advantage on Perception checks related to smell.

Spectral Bite. Melee Natural Weapon Attack, +2 to hit, 5 ft. reach, one target. On a hit, deals 2 (1d4) Necrotic damage.

Characteristics
Collective. The Grave Pack can enter and leave the space of a creature that is the same size or smaller than itself; the space within the Collective is considered difficult terrain, and the Collective has advantage on attack rolls against creatures within it.
Split. The Grave Pack can split into two Grave Packs one size category smaller using a bonus action, losing half its maximum and current hit points in the process, and its Spectral Bite has its damage dice decreased by one category. A Grave Pack can merge with another of the same size using a bonus action, adding their maximum hit points together and increasing its damage dice by one category. A Grave Pack can't be smaller than Small. A Small Pack splits into 1d4 Grave Companions.
