Part 4 · Monsters & Creatures
Unleashed Spirits
… with each spirit, a new victim entranced by the relic. They wander in silence, awaiting the next greedy fool. Unleashed Spirits are incorporeal, ephemeral specters from the Shadow Isles, contained and controlled through a Haunted Relic, a cursed item capable of imprisoning these creatures.
The exact origin of these beings is unknown, and although some aspects of their forms vaguely resemble humanoids or statues, nothing confirms who or what they may have been in life, if they were ever alive at all. It’s possible that these creatures are reflections of those who, in the past, used the relic.

Characteristics
Shifting. With a free action at the start of its turn, the Unleashed Spirit can become intangible, gaining immunity to simple physical damage, resistance to Force damage, and immunity to the Paralyzed and Petrified conditions, but becoming unable to deal any physical damage, and can return to its tangible form at the start of its next turn. While in its intangible form, the Unleashed Spirit becomes vulnerable to Force damage.
Seven Feet from Death. The Unleashed Spirit doesn't have Hit Points like other creatures; instead it has 7 vitality tokens and a damage threshold of 6. Whenever it takes damage equal to or greater than its damage threshold, the Unleashed Spirit loses one vitality token. For every 3 vitality tokens lost, the Unleashed Spirit gains +1 to AC, saving throws, attack rolls, and damage.
Ethereal Body. The Unleashed Spirit can use an action to become invisible until it takes damage, makes an attack roll, or forces a saving throw.
Descycle. When its vitality tokens are reduced to 0, the Unleashed Spirit must make a Constitution check with a DC equal to 10 or half the damage taken, whichever is higher. On a success, the Unleashed Spirit returns with 1 vitality token; on a failure, it returns to its plane or place of origin. This effect isn't triggered if it's destroyed as a result of sources that directly interact with the destruction of undead, such as "Liturgy: Destroy Undead" from a Spiritual Rite Acolyte.
Mist Wraith. This undead is native to the Black Mist and is positively affected by it.
Last Breath (Reaction). When it takes damage that would reduce its vitality tokens to 0, the Unleashed Spirit can use its reaction to negate that damage and rise again. When doing so, it can choose to reduce its damage threshold by an amount of its choice, causing a necrotic explosion in a 15-foot radius around it. Creatures in the area must succeed on a DC 10 Constitution saving throw or take a number of d8 Necrotic damage equal to the value reduced from its damage threshold, and become Paralyzed until the end of their next turns.
